St. John's requires us all to Digitally sign this waiver for EACH meeting.  The Check-In list is kept so that we can contact you in the rare case that we find out that our meeting was exposed to the covid virus.
NOTE: Only the Celebrate Recovery Ministry Leader will have access to this information.

▶ Please Review and Accept the following PROCEDURES and WAIVER.

In keeping with the regulations and restrictions of federal, state and local governments as well as the guidelines of the Centers for Disease Control and local health agencies, St. John’s Lutheran Church would like to inform you of the risks and guidelines related to worship service attendance at this time.
 
● California has implemented state guidelines, which include the stay-at-home requirements, with exceptions for essential needs and services.
● The church has been deemed an essential service, and may resume gatherings for the purpose of worship, following strict guidelines set forth by the state, county and city government and health agencies.  
● Please keep in mind any movement outside of your home assumes some risk; a visit to St. John’s campus for the purpose of worship is no different – consider your risk before committing to attend worship in our worship venue.
● If you are at-risk as defined under government guidelines due to age of 65 or greater, a health condition or other circumstance, we encourage you to remain at home and worship with us on-line.

▶ As a function of attending this event, I agree to, and will follow the procedures below:

● Perform a self-health check before leaving home.  If you are feeling ill, have a temperature (100.4 or higher), or are exhibiting any of the symptoms related to COVID-19 (coughing, difficulty breathing, sore throat, chills, muscle aches, or loss of taste or smell), remain at home, and participate in worship on-line.

● Wear a mask throughout the time you are on campus and during the event.  

● Practice social distancing of at least 6 feet from others who are attending the event.

● Use preventive hygiene practices including washing your hands, sneezing into a tissue or elbow, and not touching your face.

● Use hand sanitizer when soap and water are not available.

● Do not use the drinking fountain.

● Follow the building entry and exit paths as shown on the posted diagrams.  Per Government guidelines, entry must only be through one point, and exit must be through another point.

⚫ I acknowledge that my 'Check-In Waiver' and temperature will be checked at the door.  Anyone without a waiver in the system or with a temperature of 100.4° or higher will not be permitted to attend.
